We want your visit to be as comfortable and comprehensive as possible. Including the consultation and examination, Dr. Attai generally will spend approximately 35-60 minutes with a new patient, depending on the condition. All patients are seen on an appointment-only basis, and we make every effort to adhere to our schedule, so that the proper amount of time may be devoted to each patient - we ask for your cooperation in this matter.  If you have any questions, do not hesitate to call our office prior to your appointment. 

For your convenience, all required paperwork is available below. If you are unable to complete your paperwork ahead of time, please plan to arrive 15-30 minutes prior to your scheduled appointment time to complete the forms. Please bring your insurance card with you as well as a picture ID - this is for identity protection and is required. 

It is extremely important that we have all of your mammograms, ultrasounds, and MRI images (films or CD) in addition to the reports. If your images were performed at the Providence St. Joseph Breast Center or at any RadNet facility (Encino Breast Care Center, Burbank Imaging Center, SanFernando Interventional Radiology), we will be able to obtain the films and reports - you do not need to bring them with you. If you have previously undergone a biopsy, please  bring a copy of the pathology report if available.
